Membership Intake Form
We kindly ask all applicants wishing to become new members to fill out the intake form below. Once your intake form has been reviewed and membership approved, you will have the opportunity to schedule a free 15 minute orientation with Donna.
Member expectations:
By completing this application, you bind yourself to reverently and with intention take each microdose, should you be accepted as a Church of Our Earth Member.
By completing this application, you promise to be receptive to contact with and guidance from Higher Powers while microdosing, should you be accepted as a Church of Our Earth Member.
By completing this application, you agree not to redistribute or otherwise share Sacramental microdoses, should you be accepted as a Church of Our Earth Member.

Church membership includes:
One free 15 minute orientation for new members.
Consultations and in-depth discussions with one of our certified ministers about creating your private psilocybin microdosing program.*
Ongoing mentorship & integration coaching.*
*Training courses, counseling and consultations will be billed at the rate listed for specific courses (when offered) or at the rate of $25/15 Minutes for time outside of monthly member meetings. Monthly meetings are free for all members and include a Q&A portion.
